Estrogen and Progesterone combined
Estrogen may be applied as estriol or estradiol. Studies suggest that estriol reduces symptoms of menopause, such as hot flashes and vaginal dryness, but with a better safety profile compared with more potent estrogens, like estradiol. Our bi-est estrogen creams offer a combination of 80% estriol and 20% estradiol.
Using progesterone can either be combined into a single cream or applied separately. The same dose of estrogen and progesterone can be taken daily (continuous HRT), or in different amounts on different schedules (sequential or cyclical HRT).
There are benefits and potential side effects to both forms of hormonal support. Estrogen only hormonal support seems to offer the best hormonal support. However, estrogen-only hormonal support can cause the lining of the uterus to become too thick — a condition called estrogen associated endometrial hyperplasia. This can increase the risk of cancer of the uterus, called endometrial or uterine cancer. The progesterone in combination hormonal support helps to prevent this condition. On the other hand, estrogen only hormonal support does not increase the risk for breast cancer whereas the combination may slightly increase the chance of breast cancer when given long term. Estrogen only hormonal support is recommended for women who don't have a uterus (typically from surgery, also called hysterectomy). Estrogen only hormonal support also appears to be safe when used less than 5 years. Longer term, adding progesterone is recommended to 'protect' the uterus. However, there are other strategies.
